TRANSITION PLANNING
Special Education Secondary Transition
Transition planning is a process mandated by the Individuals with Disabilities Education Act (IDEA 2004) for all students who have an Individualized Education Program (IEP) in K–12 education. The purpose is to facilitate the student’s move from school to post-school activities. Pennsylvania Department of Education mandates that this planning begin for students at the age of 14. Transition planning must:
- Be individualized
- Be based on the student’s strengths, preferences, and interests
- Include opportunities to develop functional skills for work and community life

Students and parents should know that there are several exciting pathways to choosing a career that best fits the individual. There is no longer a right or wrong way. How to Get Started with Transition Planning
Pennwood Cyber Charter School offers several options for students and families to learn more about their choices after high school ends:
- Monthly transition prep class
- Bimonthly parent information sessions
- Appointments with a learning support teacher or transition coordinator. To complete an interview/survey that will be included in the IEP document and meeting. This information helps the IEP team prepare for the student’s transition plan.
