Thank you for considering Oklahoma Connections Academy for your child's education. Many families have found our online school program to be a better fit for their students. The information on this page will help you prepare for enrollment.

Do you have everything you need? Because Connections Academy is a public school, parents must submit state-required information and documentation for each student. You will save time and enrollment will go more smoothly if you are prepared!

Use the Enrollment Checklist below to gather the documents you will need. The checklist lists your state's requirements, plus tips to help you find documents you may not have. Once you have your documents ready, it’s easy to create an account and complete the enrollment forms in Connexus®, our online system.

Eligibility Requirements


You must meet specific state and program requirements to attend Oklahoma Connections Academy. Student eligibility requirements are determined by the Oklahoma Department of Education. Before enrolling, review the eligibility requirements carefully to determine whether your student(s) is eligible to participate in grades K–12 for the 2017–18 school year.

Students who are currently expelled from their local school or district may not be eligible to enroll.



What Documents to Have Ready

The documents below are required for enrollment in Oklahoma Connections Academy. Please submit or transfer information from the following:


Proof of age icon

Child's Proof of Age


A copy of the official birth certificate for each child must be provided to verify age eligibility.

  • Official birth certificate
  • Passport
  • Certificate of live birth
  • Baptismal papers
  • Hospital certificate

To avoid unnecessary delays, provide a copy of the official birth certificate along with your enrollment documents.

Not Sure Where to Get That?

As a reminder, the age eligibility is as follows:

  • Kindergarten: Any student enrolling in kindergarten must be five (5) years old on or before September 1 in the school year for he or she is enrolling.
  • First Grade: Any student enrolling in first grade must be six (6) years old on or before September 1 in the school year for which he or she is enrolling.
  • Maximum Age: Any student who turns twenty-one (21) years old on or before September 1 will not be eligible to enroll in Oklahoma Connections Academy.

Proof of Residence (1 form)


You must reside in the state of Oklahoma and provide ONE acceptable proof of residency to be eligible for enrollment in Oklahoma Connections Academy. You can find out what you need below:

Not Sure What is Accepted as Proof?

  • Utility bill
    Acceptable Documents Include: gas, electric, water, home phone, internet, or cable bill that is dated within 60 days of the enrollment date and includes your name and service address
  • An original lease agreement or mortgage statement for your current residence
  • Current property tax statement
  • Payroll stub from the last 60 days
  • Letter from a government agency from the last 60 days
  • W-2 form showing your home address
  • If you do not have any of these documents, please contact the Enrollment Team at 800-382-6010 to discuss acceptable alternatives

Child’s Proof of Immunization


All Oklahoma Connections Academy students must provide certification from a licensed physician or authorized representative of the State Department of Health that the student has received or is in the process of receiving immunizations. A student may be exempt from immunizations for medical, religious or personal reasons. All exemptions require a signature and date on the Connections Academy Statement of Immunization Exemption form, which is included in your enrollment documents in Connexus.

These forms must be signed by you and, if applicable, a health care provider.

Not Sure What Immunizations Are Required?

A complete list of immunization requirements is available on the Oklahoma State Department of Health website. The document is also available in Spanish.

Please provide ONE of the following documents:

  • Connections Academy Immunization form
  • A record of your child's vaccinations
  • A record showing your child is in the process of receiving the required vaccines. Please Note: Students must complete the vaccines on schedule. You must obtain a schedule to complete the vaccines from your doctor or clinic and give a copy to the school.
  • Documentation showing the student is immune from having had the disease or diseases. For information on what is needed to document immunity from having had the disease refer to Section (f) of the Oklahoma Immunization Regulations.
Where Can I Get Immunizations for My Student?

Immunizations can be obtained from doctor’s offices, clinics, or hospitals. Pediatricians and family doctors or their nurses or medical assistants can give your student the shots he or she needs to meet the requirements for school enrollment. In addition, some large chain pharmacies offer immunizations for older children and adults.

What if I Need Assistance Paying for Vaccines?

If you need assistance paying for shots/vaccinations, you may be able to get free or reduced immunizations through the program listed below:

  • Vaccines for Children (VFC) program is a federally funded program that provides vaccines at no cost to children who might not otherwise be vaccinated because of inability to pay. Children who are eligible for VFC vaccines are entitled to receive pediatric vaccines that are recommended by the Advisory Committee on Immunization Practices.
Immunization Exemptions

A student may be exempt from immunizations for medical, religious, or personal reasons. All exemptions must be completed on the Oklahoma Certificate of Exemption, which you can download from the Oklahoma State Department of Health Immunization webpage.





Proof of Vision Screening icon

Child’s Proof of Vision Screening


The Oklahoma State Department of Health (OSDH) requires that students entering kindergarten, first grade, and third grade receive a vision screening by a qualified vision screener. The purpose of vision screening is to measure distance acuity and the quality of depth perception. According to the OSDH, "Vision Screening" means the process or system used to identify children who may be at risk of having or developing visual problems that may adversely affect their ability to acquire knowledge, skill, or learning, for the purpose of recommending further evaluation by an eye care professional. A vision screening is not the equivalent of a comprehensive eye exam."

Not Sure What Is Accepted as Proof?

If your student is entering kindergarten or first or third grade, he or she will need to complete a vision screening by a qualified vision screener, who will provide you with a certificate as proof of vision screening. Please review a complete list of qualified vision screeners.

  • Is your student a kindergartner? Please provide certification to Oklahoma Connections Academy that the student passed a vision screening within the previous twelve (12) months or during the school year.
  • Is your student a first (1st) or third (3rd) grader? Please provide certification to Oklahoma Connections Academy within thirty (30) days of the beginning of the school year that the student passed a vision screening within the previous twelve (12) months.

Prior Academic Records


All students who have previously attended public school, private school, or homeschool are eligible to enroll and will be requested to submit academic records for placement.

  • Report Card
    For students applying for grades K–9: Please submit a copy of the report card from the most recent school year.

    The following items should be included on the report card:
    • Student Name
    • Grade Level
    • School Year (example: 2016–17)
    • School Name
    • Grading Scale (example: 90–100%=A)
  • Unofficial Transcript
    For students applying for grades 10–12: Submit an unofficial transcript that includes the complete end-of-year grades from the prior school year and any academic documentation (such as a progress report/interim report, class schedule, or marking period/semester report card) issued for the current school year. Additional documentation may be requested for placement.

    The following items should be included on the unofficial transcript:
    • Student Name
    • Grade Level
    • School Year (example: 2016–17)
    • School Name
    • Grading Scale (example: 90–100%=A)
    • Credits Earned
Not Sure How to Get Your Child’s Report Card or Transcript?

  • Contact your student's previous school and request the document, providing them with the information outlined above, OR contact your school's district office with the request.

If after attempting these steps you still cannot obtain your student's report card or transcript, please contact the academic placement services team at 800-382-6010 to discuss acceptable alternatives.

Have Your Child(ren) Been Formerly Homeschooled?

If your children are currently homeschooled, please complete one of the two documents below. These documents will be available to you in Connexus.

  • Prior Academic History Form
    For homeschooled students in grades K–8: Complete the Prior Academic History Form indicating what courses were completed through a homeschool curriculum.
  • High School Homeschool Credit Form
    For homeschooled students in grades 9–12: Complete the High School Homeschool Credit Form indicating any high school level courses completed through a homeschool curriculum.

Though not applicable to all families, please submit the documentation requested if the information below is specific to your student.

Limitations and Other Documentation


You may need to provide additional documentation if any of the following circumstances apply:

  • Custody order: If your legal guardianship is determined by a court order, please submit a copy of the order.
  • Special Education Documentation: If your student has had an Individualized Education Program (IEP) within the past three years, please submit the following documents:
    • IEP Documentation: Please submit a copy of the most recent IEP from within the last year.
    • Supporting Evaluation Documentation: Please submit a copy of the most recent Multidisciplinary Evaluation and Eligibility Group Summary (MEEGS) from within the last three years.
  • Additional Special Education Documentation:
    • IEP Exit Documentation: If your student has been formally exited from special education services, please submit a copy of the formal exit documentation.
  • 504 Plan: If your student has had a 504 Plan within the past three years, please submit a copy of this document.
  • State Test Scores: If your student is applying for fourth grade, the reading portion of the state assessment is used for promotion criteria and will be reviewed by an academic placement advisor during the course placement process. Please submit a copy of the results for this assessment to expedite your student’s course placement.
