- First semester enrollment is open and capacity is limited to 1,250 students.
- Once the capacity is met, students who complete all enrollment tasks will be placed on a waiting list and released to enroll on a first come, first served basis

Enrollment Information for the 2024-25 School Year
Per state law enrollment is limited.
If the number of applications accepted exceeds the capacity limit, we will use a lottery-style drawing to select students for enrollment on 3/26/2024
To be eligible for the lottery, please start registration and follow the steps to create an application.
If the school does not exceed its enrollment limit, no lottery is required. You will then be able to continue enrolling your student.

- The enrollment capacity is 1,250 students.

Grades K-8 Deadlines
Grades K-8 will close enrollment for the 2023-24 school year at 5 pm MT on 3/1/2024.
- The last day to register: 2/23/24
- The last day to submit: 2/26/24
- The last day to complete Final Confirmation: 3/1/2024 at 5 pm MT

Check Eligibility
You must meet specific state and program requirements to attend Utah Connections Academy. Student eligibility requirements in Utah are determined by the Utah State Office of Education. Before enrolling, review the following eligibility requirements carefully to determine whether your student(s) is eligible to participate in grades K–12. Once your enrollment is confirmed, please immediately withdraw your student from their previous publicly-funded school. Students cannot be simultaneously enrolled in two public schools.
Note: Utah Connections Academy does not accept students who are currently expelled from their local school or district. Students who are currently suspended from their local school or district may not be eligible to enroll and will be reviewed on a case-by-case basis.

A copy of one of the following documents must be provided for every student to verify age eligibility. To avoid unnecessary delays, provide a copy of this document along with your enrollment documents.
- Official birth certificate
- Passport
- Certificate of live birth
- Hospital certificate

- If your student was born in Utah and you need a copy of his or her birth certificate, you can order it online through the Utah Department of Health. Please visit the "How to Order a Birth Certificate" webpage for more information.

As a reminder, the age eligibility is as follows:
- Kindergarten: Any student enrolling in kindergarten must be five (5) years old on or before September 1 in the school year for which you are enrolling.
- First grade: Any student enrolling in first grade must be six (6) years old on or before September 1 in the school year for which you are enrolling.
- Maximum Age: Any general education student who turns eighteen (18) years old on or before September 1 will not be eligible to enroll. Any special education student who turns twenty-one (21) years old on or before September 1 will not be eligible to enroll.

Proof of Residence
You must reside in the state of Utah and provide acceptable proof of residency to be eligible for enrollment in Utah Connections Academy. There are many verifiable forms of documentation, which are detailed below.
Not Sure What is Accepted as Proof?
- Gas, electric, water, cable, telephone, internet, or sewage utility bill from the last 60 days, showing your name and service address
- Mortage statement or current signed lease agreement
- Most recent property tax statement*
- Voter registration card
- Valid Utah Driver's license or other Utah photo ID
- Bank statement
- Rent receipt
- Property deed
- Letter from a government agency
- Payroll stub
- Credit card statement
- Certificate of tribal enrollment or other identification issued by a recognized Native American tribe

Child’s Proof of Immunization
All Utah Connections Academy students must provide evidence of their immunizations. A student who, at the time of school enrollment, has not been completely immunized against each specified disease may attend school under a conditional enrollment if the student has received one dose of each specified vaccine prior to enrollment and is completing the required immunizations as allowed by the medically required time periods between doses. Therefore, a student who enrolls under a conditional enrollment must complete all required immunizations within the designated period or be excluded from school.
Students who transfer from another state or from one Utah school to another must provide appropriate immunization documentation that satisfies Utah’s requirements.
The Utah School Immunization Record (USIR) is the official certificate of immunization for students in any Utah public, private, charter or parochial school. The USIR is commonly referred to as the "Pink Card" and is part of the student’s permanent school record.
Please submit one of the following:
- A Utah School Immunization Record (Pink Card)
- If the Utah School Immunization Record is not signed by a licensed physician, registered nurse, or public health official, it must be accompanied by a current copy of the student’s shot record signed by a licensed physician, registered nurse, or public health official.
- Any immunization record signed by a licensed physician, registered nurse, or public health official.
Not Sure What Immunizations Are Required?
To enroll in kindergarten, a student must have written proof of receiving immunizations listed on the "Immunization Requirements for Kindergarten Entry 2023–24" webpage.
To enroll in the seventh (7th) grade, a student must have written proof of receiving immunizations listed on the "Immunization Requirements for 7th Grade Entry 2023–24" webpage.
Where Can I Get Immunizations for My Student?
Immunizations can be obtained from doctor’s offices, clinics, or hospitals. Pediatricians and family doctors or their nurses or medical assistants can give your student the shots he or she needs to meet the requirements for school enrollment. In addition, some large chain pharmacies offer immunizations for older children and adults.

What if I Need Assistance Paying for Vaccines?
If you need assistance paying for shots/vaccinations, you may be able to get free or reduced immunizations through this program:
- Vaccines for Children (VFC) program is a federally funded program that provides vaccines at no cost to children who might not otherwise be vaccinated because of inability to pay. Children who are eligible for VFC vaccines are entitled to receive pediatric vaccines that are recommended by the Advisory Committee on Immunization Practices.
Immunization Exemptions
A student may be exempt from immunizations for medical, religious, or personal belief reasons. To receive a vaccination exemption form, the legally responsible individual of a student must complete an online education module (free of charge), print the vaccine exemption form, and present the form to the school.
If the legally responsible individual declines to complete the online education module, he/she must receive an in-person consultation at the local health department ($25.00 fee may apply), sign the exemption form at the local health department, and provide a copy of the completed school immunization exemption form to the school official.
For a medical exemption from vaccination, the legally responsible individual of the student must present to the school a completed vaccination exemption form and a written notice signed by a licensed healthcare provider stating that due to the physical condition of the student, administration of the vaccine would endanger the student’s life or health.

Students across Utah in grades K-12 are eligible to enroll in Utah Connections Academy. Those who have previously attended public school, private school, or homeschool will be asked to submit academic records for placement.
- Report Card
For students applying for grades K–9: Please submit a copy of the report card from the most recent school year.
The following items should be included on the report card:Student Name
- Student Name
- Grade Level
- School Year (example: 2023–24)
- School Name
- Grading Scale (example: 90–100%=A)
- Unofficial Transcript
For students applying for grades 10–12: Submit an unofficial transcript that includes the complete end-of-year grades from the prior school year(s) and any academic documentation (such as a progress report/interim report, class schedule, or marking period/semester report card) issued for the current school year. Additional documentation may be requested for placement.
The following items should be included on the unofficial transcript:
- Student Name
- Grade Level
- School Year (example: 2023–24)
- School Name
- Grading Scale (example: 90–100%=A)
- Credits Earned
Not Sure How to Get Your Child’s Report Card or Transcript?
- Contact your student's previous school and request the document, providing them with the information outlined above, OR contact your school's district office with the request.
If after attempting these steps you still cannot obtain your student's report card or transcript, please contact the academic placement services team at 800-382-6010 to discuss acceptable alternatives.
Have Your Child(ren) Been Formerly Homeschooled?
- Prior Academic History Form
For homeschooled students in grades K–12: Complete the Prior Academic History Form indicating what courses were completed through a homeschool curriculum. This form will be available in the Enrollment Portal. - Homeschool Affidavit
For homeschool students in grades K–12: In the state of Utah you should have submitted an affidavit informing your school district that your child attended a homeschool. Please submit a copy of your homeschool affidavit.
