Thank you for considering Oregon Connections Academy for your child's education. Many families have found our online school program to be a better fit for their students. The information on this page will help you prepare for enrollment.

Get Prepared for Enrollment Information


Do you have everything you need? Because Connections Academy is a public school, parents must submit state-required information and documentation for each student. You will save time and enrollment will go more smoothly if you are prepared!

Use the Enrollment Checklist below to gather the documents you will need. The checklist lists your state's requirements, plus tips to help you find documents you may not have. Once you have your documents ready, it’s easy to create an account and complete the enrollment forms in Connexus®, our online system.

Eligibility Requirements


You must meet specific state and program requirements to attend Oregon Connections Academy. Student eligibility requirements in Oregon are determined by the Santiam Canyon School District, which is the authorizing body for Oregon Connections Academy. Before enrolling, review the eligibility requirements carefully to determine whether your student(s) is eligible to participate in grades K–12.




What Documents to Have Ready

Please have the following documents available and ready to upload to Connexus:

The documents below are required for enrollment in Oregon Connections Academy. Please submit or transfer information from the following:

Child's Proof of Age


A copy of one of the following must be provided for each child in order to verify age eligibility:

  • Official birth certificate
  • Passport
  • Certificate of live birth
  • Baptismal papers
  • Hospital certificate
  • Custody documentation
  • Military ID or Driver’s License
  • Adoption record
  • Green card

To avoid unnecessary delays, provide a copy of one of the items listed above along with your other enrollment documents. If you do not have an acceptable document, please contact the enrollment team at 800-382-6010.

Not Sure Where to Get That?

As a reminder, the age eligibility is as follows:

  • Kindergarten: Any student enrolling in kindergarten must be five (5) years old on or before September 1 in the school year for which he or she is applying.
  • First Grade: Any student enrolling in first grade must be six (6) years old on or before September 1 in the school year for which he or she is applying.
  • Maximum Age: Any general student who turns nineteen (19) years old on or before September 1 will not be eligible to enroll. Any special education student who turns twenty-one (21) years old on or before September 1 will not be eligible to enroll.

Proof of Residence (1 form)


You must reside in the state of Oregon and provide ONE acceptable proof of residency to be eligible for enrollment in Oregon Connections Academy. You can find out what you need below:

Not Sure What is Accepted as Proof?

  • A residential utility bill (gas, electric, or water), Internet, propane, or trash bill from the last 60 days, that includes your name (as the resident) and your service address
  • A mortgage statement or annual lease agreement
  • Property tax statement
  • Letter from a government agency from the last 60 days
  • If you do not have any of these documents, please contact the enrollment team at 800-382-6010 to discuss acceptable alternatives.

Geographical Limitations


As a reminder, you must reside in the state of Oregon and provide acceptable proof of residency to be eligible to enroll in Oregon Connections Academy. Students do not need to be withdrawn from their previous school prior to enrolling. Please complete the placement process and Oregon Connections Academy will notify the student's previous district of the change in schools. HB2301 limits enrollment in virtual schools for full-time students to no more than 3% of a school district’s population. Rules will be issued as to how this will be measured and how students will be notified if they exceed the limit. We do not expect that the limit will impact students enrolling for the 2017–18 school year but will notify any students who are affected.

Child’s Proof of Immunization


All Oregon Connections Academy students must provide evidence of their immunizations on the Oregon Certificate of Immunization Status Form. This form must be signed by you and, if applicable, a health care provider.

Oregon allows for religious, medical, or immunity exemptions; however, medical and immunity exemptions must be verified by a health care provider. Please use the Oregon Certificate of Immunization Status Form if requesting an exemption.

Where Can I Get Immunizations for My Student?

Immunizations can be obtained from doctor’s offices, clinics, or hospitals. In addition, some large chain pharmacies offer immunizations for older children and adults.

In the state of Oregon, most pediatric and family practice clinics offer immunizations. If your child has a regular medical provider, we recommend calling that office for an appointment for immunizations. If your child does not have a regular provider, you can contact your local county health department, or call 1-800-SAFENET (1-800-723-3638). They will be able to help you find a clinic where your child can be immunized.

What if I Need Assistance Paying for Vaccines?

If you need assistance paying for shots/vaccinations, you may be able to get free or reduced immunizations through the program below:

  • The Vaccines for Children (VFC) program is a federally funded program that provides vaccines at no cost to children who might not otherwise be vaccinated because of inability to pay. Children who are eligible for VFC vaccines are entitled to receive pediatric vaccines that are recommended by the Advisory Committee on Immunization Practices.

Immunization Exemptions


A student may be exempt from immunizations for the following reasons: religious, medical, or immunity.

  • Religious Exemption: According to the OHA, Oregon recognizes parent-signed exemptions from vaccine requirements for religious reasons. "Religion" is defined in Oregon Administrative Rule 333-050-0010 as a system of beliefs, practices, or ethical values. An exemption may be claimed for one or more vaccines or for all vaccines.   • Medical Exemption: Medical exemptions must be verified by a physician or the county health department.

Prior Academic Records


All students who have previously attended public school, private school, or homeschool are eligible to enroll and will be requested to submit academic records for placement.

  • Report Card
    For students applying for grades K–9: Please submit a copy of the report card from the most recent school year.

    The following items should be included on the report card:
    • Student Name
    • Grade Level
    • School Year (example: 2016–17)
    • School Name
    • Grading Scale (example: 90–100%=A)
  • Unofficial Transcript
    For students applying for grades 10–12: Submit an unofficial transcript that includes the complete end-of-year grades from the prior school year and any academic documentation (such as a progress report/interim report, class schedule, or marking period/semester report card) issued for the current school year. Additional documentation may be requested for placement.

    The following items should be included on the unofficial transcript:
    • Student Name
    • Grade Level
    • School Year (example: 2016–17)
    • School Name
    • Grading Scale (example: 90–100%=A)
    • Credits Earned
Not Sure How to Get Your Child’s Report Card or Transcript?

  • Contact your student's previous school and request the document, providing them with the information outlined above, OR contact your school's district office with the request.

If after attempting these steps you still cannot obtain your student's report card or transcript, please contact the academic placement services team at 800-382-6010 to discuss acceptable alternatives.

Have Your Child(ren) Been Formerly Homeschooled?

If your children are currently homeschooled, please complete one of the two documents below. These documents will be available to you in Connexus.

  • Prior Academic History Form
    For homeschooled students in grades K–8: Complete the Prior Academic History Form indicating what courses were completed through a homeschool curriculum.
  • High School Homeschool Credit Form
    For homeschooled students in grades 9–12: Complete the High School Homeschool Credit Form indicating any high school level courses completed through a homeschool curriculum.

Though not applicable to all families, please submit the documentation requested if the information below is specific to your student.

Other Documentation


You may need to provide additional documentation if any of the following circumstances apply:

  • Dental Screening: Any student seven (7) years old or younger is required to have a dental screening or show proof they've had one.
  • Custody order: If your legal guardianship is determined by a court order, please submit a copy of the order.
  • Special Education Documentation: If your student has had an Individualized Education Program (IEP) within the past three years, please submit the following documents:
    • IEP Documentation: Please submit a copy of the most recent IEP from within the last year.
    • Supporting Evaluation Documentation: Please submit a copy of the most recent Three-year Psychological Education Report.
    • Eligibility Determination: Please submit a copy of the most recent Eligibility Page.
  • Additional Special Education Documentation:
    • IEP Exit Documentation: If your student has been formally exited from special education services, please submit a copy of the formal exit documentation.
  • 504 Plan: If your student has had a 504 Plan within the past three years, please submit a copy of this document.
